Form 1095-C Availability
LSU Health Sciences Center New Orleans provides Form 1095-C in accordance with IRS requirements. Active employees may access their Form 1095-C electronically through Employee Self-Service.
Paper copies of Form 1095-C are not automatically mailed. However, any employee or retiree may request a paper copy of their Form 1095-C at no cost. To request a paper copy, please email NOHRMBenefits@lsuhsc.edu or call 504-568-7780.
Requested forms will be furnished no later than January 31,2026, or within 30 days of the request, whichever is later. Additional information about Form 1095-C is available on the IRS website.
